Apparently it occurs if Ali headbutts in the middle of the dash
It was alrdy reported to riot

I think it will go to the end of the map according to whitch direction she is headbutted , like if you headbutt her to your base she will die

I know it's a bit of an old one, but actually this one does not have anything to do with Alistar as far as I know.
I recently encountered the same thing (and I even have a replay of it, if someone wants to check it). You always fly to your fountain. It's really weird. I think it's about using your ultimate at a very specific time. As you can see, there was a purple square on the ground, so she did use her ult.
I can vaguely remember, that I used my Q, dashed forward but already pressed R and then started flying towards 0/0 coordinates.
